PPP Forms
Forms required after acceptance to the program:
1) TB Form (Required): TB Test is required for volunteering in class and school. Please see your doctor to obtain a copy of clearance. TB test clearance are good for four years.
2) Request to use private vehicle for transporting students Form (Required): In order to qualify to transport students in private vehicle, this form must be filled out and submitted to your teacher. In addition, your insurance company MUST send a "Certificate of Insurance", faxed directly to the attention of Principal Ortiz at fax number (408) 937-5006 (see attached form).
Note: If you changed your vehicle during school year, please update the new form and Certificate of Insurance with school.

3) Donation Form (2022-2023 School Year): At the onset of the school year, teachers provide each family with a classroom budget proposal for the school year, which includes an itemized description of expenditures and estimated budget. Expenditures includes classroom materials and supplies, outdoor field trips, technology and other enrichment programs.
If your employer sponsors a company matching program, please indicate it on the donation form. We encourage families to tap into these company benefits for your kids in the classrooms. Please work with your company’s Human Resource Department to kickstart the process early and submit the form to them at your earliest convenience as past experience has shown us that this process may take a long time (see attached form for details).
